Immigration Medical Examination
Civil Surgeon
The immigration medical exam required by U.S. Citizenship and Immigration Services needs to be completed by a designated physician. These approved doctors are also known as Civil Surgeons, which we have on site at Infinite Results.
What To Bring
- Valid, Government Issued Photo ID
- Vaccination Records (if available)
- Payment: Cash and Credit Cards are accepted
- Download, Fill Out Part 1 and Print the I-693 Form before coming in:
Completed Immigration Medical Exam
The civil surgeon will complete, sign, and seal the I-693 Form and any supporting documents in an envelope. You must submit the sealed envelope to USCIS as directed in the Form I-693 instructions.
